Since its inception, the Foundation has raised close to $4.0 million through individual contributions and educational grants.

 

 

 

Congratulations to any teacher receiving a grant from the Foundation.

As stated on the grant application, the Foundation requires an evaluation to be completed by each of its awardees. The program evaluations enable the Foundation to assess the success of the programs it has chosen to support.  Thank you for taking the time to reflect on your project!

Evaluations should contain the following information:

1. Program Title and name of person who submitted the application, as well as contact telephone number and e-mail address.  Include actual program dates or timeline.

2. Name of person completing the evaluation (if different from the person who submitted the proposal). INCLUDE ALL CONTACT INFORMATION, including phone number and e-mail address.

3. Analysis of  the effectiveness of the project's activities and whether the goals and objectives of the project were met. PLEASE BE SPECIFIC, rather than general.

4. Description of any information that was gathered and recorded before, during and after the project.

5. Student assessments, where appropriate.  These are particularly useful.

6. A summary of actual grant expenses.
