How to sign up for an appointment to receive the vaccine

YOU MUST HAVE AN APPOINTMENT TO RECEIVE THE VACCINE.

Because the list of those who are eligible changes, you should first confirm that you are eligible. You can do so by checking on this web application from New York State: https://am-i-eligible.covid19vaccine.health.ny.gov/.

If you are eligible, the same web application can direct you to information to help you set up an appointment. This will be the appointment for your first dose of the vaccine. You will also need to receive a second dose of the vaccine. You should be given an appointment for your second dose at the site where you receive your first dose.

If you have problems with the online site, as of 4pm today, there will also be a hotline available to help schedule appointments for eligible New Yorkers: 1-833-NYS-4-VAX (1-833-697-4829).

Prior to receiving the vaccination, you must complete the New York State COVID-19 Vaccine Form. This form can be completed online at https://forms.ny.gov/s3/vaccine

If you are eligible for an appointment, you must bring proof of eligibility to the vaccination site. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If you are eligible because of your age, bring a government-issued ID that includes your date of birth (like a Driver’s License or passport).

For instructions on how to use the eligibility app, check out the online instructions

Please know that even if you call today, your appointment may be scheduled for several weeks from now. For those over the age of 75, designated pharmacies as well as the County Center are current distribution sites in our region.

Volunteer to help with vaccine distribution:

Westchester Medical Center is the coordinating hub for our region. They are calling for volunteers to help with vaccine distribution:

"Volunteer to Support COVID-19 Vaccination

How To Volunteer

As the coordinator for the Hudson Valley Regional HUB, WMCHealth is seeking volunteers to support the administration of the COVID-19 vaccine.

Volunteers are needed to assist with operations and activities at vaccination sites, such as registration and check-in, data entry, greeting and routing participants, traffic flow, and other administrative tasks. Volunteers with specific clinical credentials to administer the vaccine are also needed.

Volunteers must be 18 years of age in good health and proficient in English, with bi-lingual skills also being important. Those working at the vaccination site will be provided with training, PPE and will be able to receive the vaccine, if they wish.

Site locations will be announced in the coming days and will operate 7 days a week. Multiple shifts are available. Volunteers are asked to commit to three shifts a week during a 30-day commitment.
